Returned Check, Credit Card Denied/chargeback, Collection, and Insurance information policy

If a check is returned for any reasons, a returned check fee of $25 per check is assessed on the patient. This is assessed by the bank and hence we are not able to waive it off.

If credit card on file is denied the correct charges, a rebilling fee of $30 per bill denied is automatically added. This is the charge from the clearing house and we are not able to waive it off.

If a bill is unpaid even after due date a uniform $5 late fee per month is added. 

If a bill is unpaid even after 3 collection attempts through phone/sms/email/mail it is handed off to a collection agency and a collection agency charge of $100 is added to your bill to cover our costs and collection agency may add more charges as per their policy.

We are not responsible for the actions of debt collection agencies. Once the bill is into collection, your chart gets marked as "in collection" and remains in your records. We are not able to remove that because charts are automatically uploaded into Health Information Exchange. 

Debt collection agencies can report debts to insurance carriers, particularly in cases of unpaid premiums, deductibles, or other outstanding balances related to insurance claims. Insurance carriers may use this report to deny past and future claims. 

Please never allow your account to go into collection.
