Prescription Transfers

Many patients call up after the prescription is already sent asking to change the pharmacy.  Pharmacy change can happen only at the beginning of the visit. If a prescription has already been sent, then they must get the transfer done from the pharmacy themselves.  They can go to any other pharmacy and request transfer. No transfer possible from the clinic.

 

Controlled Substances

There is no refill for controlled substances.  Each prescription is considered as a new prescription. Please do not call us and seek a refill over phone. No refills will be made.  

 

Addiction Treatment

If you are getting treated for addiction, all medicines with addiction potential must be filled by your addiction treatment specialist.

Stolen Prescriptions

If a prescription is stolen then no new prescription possible to a new pharmacy. It will be called in to same pharmacy where the prescription was stolen after the patient has filed a police complaint and pharmacist agrees that medicines can be dispensed again. Pharmacist can send a fax requesting a new prescription after verifying police complaint.  This fax must be sent to 720.216.1934. Only after pharmacist confirms that police complaint has been filed and patient is justified in seeking new prescription a new prescription for the amount of medicines stolen will be called in. 

 

 

Information

At the time of office visit, please present your insurance card.  We bill your visit to the insurance card you present. 

In the event of an error in your insurance, please let our front office staff know.

We are in network with most insurances. However individual plans may vary and not known till a claim is adjudicated.

We are a smoke free facility.

We need every patient to finish all the forms online and check in online if possible. 

Most visits start on time, so please plan to be at the office, fifteen minutes before your appointment.

Habitual no show patients are terminated from the patient panel.

We charge a no show fee.
