Colorado Medicaid has assignments. Assignments are in place to reduce the cost and improve outcomes. Medicaid asks your input before they assign you to a plan.
When you are assigned a plan and if we are not in your plan Medicaid does not pay. If you are not in our plan and still want to see us use the following FAQ to get yourself help.
I am not in the plan. Can I still see a provider in New Horizons Primary Care?
Visit enroll.healthfirstcolorado.com or call Health First Colorado Enrollment at 1-888-367-6557. State Relay: 711 for callers with hearing or speech disabilities. Call Monday to Friday, 8 a.m. to 5 p.m. The call is free. Ask them to assign you to one of the plans of Colorado Access. Once your are in Colorado Access ask them to assign New Horizons Primary Care as your PCP. It takes one month for Medicaid to change plan and PCP. If you have immediate issue see your current primary care. Ask Medicaid if you do not know who it is. Once you have requested change, book your appointment with us in the following month. It takes a month for Medicaid to change the plans.
Insurance Issues - Commercial
As healer our focus is you. We do not want to be referee between you and your insurance. If your insurance is not paying for the visit at the time of office visit, you provide guarantee for the payment either by paying full amount at the time of visit or putting a credit card on file.
We make every attempt to understand your network and plans. However when insurance companies do not share full details with you, they even share less details with us. If you have problem with billing, coverage or questions about insurance, call your insurance. You can also drop the insurance and use super affordable self pay rates for all the primary care and use insurance only at the hospitals.
