The information we collect from you, to the extent that it is private, is processed in accordance with this Privacy Policy.

3.1. Using App or Site

Identifying Information

We collect certain personal information from you when you use the Service, that can be used to identify you, namely IP address, device ID and cookie information. Such personal information is only used for the technical provision of the Service and to collect aggregated statistics on the actual usage. Providing such personal information is a contractual requirement to use the Service. By using the Service, you give consent for processing of the personal information.

The Service doesn’t require any form of registration or sign up and we never your e-mail, name or contact information.


We may collect and store information about your location if you enable your computer or mobile device to send us location information. You may be able to change the settings on your computer or mobile device to prevent it from providing us with such information.

Non-Identifying Information

Whenever you visit the Service, we may collect non-identifying information from you, such as referring URL, UID, location information, browser, operating system and Internet Service Provider. Health information

The Service collects health information such as gender, age, input we might receive from you when interacting with the chat bot, symptoms and risk factors. Such health information is not connected to any of your personal information and is fully anonymous. We link this information with the anonymized unique identifier to gain insights into statistical data that help us improve the Service.

4.  Use of Your Information

We may solely use your information to:

Enhance or improve User experience, our Site, our App, our Chat bot or our Service.

Perform any other function that we believe in good faith is necessary to protect the security or proper functioning of our Site or Service.

5.  Accessing, Editing, and Removing Your Information

Users have a right of access and erase their personal data. Users who wish to perform any operations on their personal information are requested to share their requests via email to admin@nhpc.us.

We take no responsibility for stored information in your cache, or in other devices that may store information, and disclaim all liability of such.  In addition, we may, from time to time, retain residual information about you in our backup and/or database.

8.  Third Party Access to Your Information

We do not rent, share, sell or trade personal information with third parties for their direct marketing purposes. That said, we work with third parties and they may have access to some of your information:

8.1 Service Providers

We outsource some of our tracking and reporting functions (e.g. Google Analytics, Amplitude) and other services to third parties. We may share information from or about you with them so that they can perform their services.

8.2 Business Transfers

We may share information from or about you with subsidiaries, shareholders, joint ventures, or other companies under common control, in which case we will require them to honor this Privacy Policy. If another company acquires New Horizons Primary Care or all or substantially all of our assets, that company will possess the same information, and will assume the rights and obligations with respect to that information as described in this Privacy Policy, unless we notify you otherwise.

9.  Release of Your Information for Legal Purposes

At times it may become necessary, for legal purposes, to release your information in response to a request from a government agency or a private litigant.  You agree that we may disclose your information to a third party where we believe, in good faith, that it is desirable to do so for the purposes of a civil action, criminal investigation, or other legal matter.  In the event that we receive a subpoena affecting your privacy, we may elect to notify you to give you an opportunity to file a motion to quash the subpoena, or we may attempt to quash it ourselves, but we are not obligated to do either.  We may also proactively report you, and release your information to, third parties where we believe that it is prudent to do so for legal reasons, such as our belief that you have engaged in fraudulent activities.  You release us from any damages that may arise from or relate to the release of your information to a request from law enforcement agencies or private litigants.

We take certain measures to enhance the security of our Site and Service, including using SSL Certificates.  However, we make no representations as to the security or privacy of your information.  It is in our interest to keep our website secure, but we recommend that you use anti-virus software, firewalls, and other precautions to protect yourself from security threats.

11.  Your California Privacy Rights

New Horizons Primary Care PC permits residents of California to use its services.   If you are a California resident, you may request certain information regarding our disclosure of personal information to any third parties for their direct marketing purposes.  Various provisions throughout this Privacy Policy address requirements of the Californian privacy statutes. New Horizons Primary Care PC is a covered entity as per Health Insurance Portability and Accountability Act (“HIPAA”).  We comply with provisions of HIPAA.  You are hereby informed of the following:

You may request the information we hold about you from us and we will provide it to you within thirty days of your request.

We may disclose your information to third parties for health-related or marketing purposes.  You hereby authorize us to make such disclosures.

New Horizons Primary Care PC’s employees do not actively monitor customer information.  Rather, it is automatically processed when you choose to input it.  We do, however, restrict who may access our servers.

13.  Minors

Individuals under 18 years of age are not allowed to use our Service.  If you become aware that a User who is under the required age has used all or any portion of our Service, please notify us immediately at admin@nhpc.us, and provide us with full details as to why you believe they are below that age, and we will address the issue.  If you are a User who is reported in this manner, we may require you to provide suitable proof of age, such as a copy of government identification, in order to continue using our Site and/or Service.

14.  International Transfer

Your information may be transferred to and maintained on computers located outside of your state, province, country or other governmental jurisdiction where the privacy laws may not be as protective as those in your jurisdiction. Your consent to this Privacy Policy followed by your submission of such information represents your agreement to that transfer.

15.  Amendments

Like our Terms of Service, we may amend this Privacy Policy from time to time. When we amend this Agreement, we will place a note on our Site as to when this Privacy Policy was last modified, and we will e-mail you about it.  If you do not agree, you must cease using our Site and Service immediately.
